I’m sure someone’s thought this up before, but…navigation. Namely, needing to keep track of north.
Easy way is to rotate the map randomly when sleeping or otherwise distracted, based on survival skill. Hard way is to build player facing and rotation into the game SOLELY to make the interface more confusing.
A compass obviously could be checked to reorient yourself. You could also obtain wearable compasses to eliminate navigation issues entirely. Though since most wearable compasses I’ve seen are shitty button compasses that are either A: air-filled and thus very swingy or B: put together so badly that they have to be tapped on a few times due to getting stuck, you could of course have low-quality compasses that reduce, not eliminate, the chance of getting disoriented.
You could also have electromagnetic effects mess with the compass’s ability to function, or even have metal armor or other gear possibly fuck with it.

Guys, seriously. We really need Zombie Scientists to spam more manhacks. Some of my lab scenario starts are encountering rooms that have tiles that don’t have manhacks on them, and that’s just not acceptable.
