Only in melee–guns shouldn’t fire any faster because your body is sped up.
That wouldn t change the outcome though.

This is actually a serious idea, but undoubtedly also horrible:
Eating is a big part of most roguelikes, and Cataclysm makes sleeping a big thing. The next logical biological function is…pooping and peeing.
You could have a bladder indicator, telling you had badly you need to pee/poop. Particularly dangerous encounters and drugs could make you have to crap immediately or piss yourself, and bad food could give you uncontrollable diarrhea. Pee could be used in emergencies instead of water (maybe boil it to make it safer), and poop could give off powerful scent clouds to attract zombies, and be usable as a thrown weapon or fertilizer for farming.
(Warning you now, that one has actually been kicked around a few times, for better or for worse. Mostly worse, so far.)

I would be content with shocker and spitter bacteria. The shocker bacteria would sometimes turn up as colonies of dozens of distinct entities in the wild. Spitter bacteria would just live on random tiles, and only be detected when they release acid, when they are disturbed, usually by having items dropped on them… Of course both would be distributed around the environment and would infect survivors at random…
A very small random chance of dying from a heart attack when you get hit in the chest.
If its cold enough there should be a chance of the water freezing instantly killing you when you dive into water.
