When giving you followers a weapon they’ll sometimes have a better weapon but either not enough skill to make full use of it or you plainly want those weapon. You just trade their weapon away anyways, so I think it should be that you’d be prompted like when they have a conflicting armor with what you want them to wear, and be given a choice to either have them drop it or keep it.
My best practice for this is get naked, get the npc naked. Dress up yourself with everything you want them to use and just hand it all over.
NPC’s are dummies anyway just dress them up once and be done.

I give my follower a good melee weapon and they just throw it at the zombie and then take off their armor to use as a bashing weapon, probably leading to them throwing that as well.

Never had that happen, might need to force them to use melee only, attack without moving or else update the game.
