If you’re speaking about in game, then sure. It’s personal preference, really.
In real life however, there have been plenty of links between drug abuse and mental disorders.
Even daily use of marijuana (for years and years, decades even (especially before the age of eighteen, apparently)) has been linked to schizophrenia, though it’s difficult to say whether it simply triggers something that has always been there or whether altering the chemistry of your brain on a daily basis actually causes it. Certainly people with a family history of mental illness should stay away from drug abuse.
I don’t condone or condemn recreational drug use (honestly, it’s a person to person thing like with alcohol. Some can handle their drink, others become alcoholics), but there IS a marked difference between use and abuse. It’s the same for everything you put in your body: moderation is key.
If you’d really like I can dig out some citations for you, but it’d be easier if you did a simple search on Google for it. Or just take my word, I guess. Critical thinking is important though, so I recommend that if you’re interested you actually do some first hand research. Also, keep in mind that this information doesn’t necessarily apply to you, and you may actually have anecdotal evidence that contradicts it, but we’re talking scientific studies and statistics taken from a wide range of people here.
Sorry to have taken this so seriously, but it’s important to be aware of all the facts when dealing with your mental health.
I think there ought to be clean versions of drugs and unclean versions, like the clean and unclean waters. Only with more ramifications in game. Like getting sinus infections, chest damage, and so on. You could wash drugs with acetone in game.

Also, also, sudden thought, perhaps a chance to get diseases like HIV from a needle?
HIV, for example, make your disease resistance level down over time (Except for disease immune, which is a fanciful trait that could save you from this?) in order such as Disease Resistant (if you have it), traitless as the average person, and then make further downwards traits such as disease susceptible, etc etc, the lower and lower ones making diseases MUCH more frequent, and the effects that aren’t immunoresponses would be worsened drastically. Perhaps even cause new diseases that would happen even in the most healthy of situation, but never occurs to non HIV patients.
I can see something like that common to make someone want to quit their game a lot though, unless they get a mutagen to strike lucky on disease immune, in when they’ll never drink another purifier again just so they won’t die.

You must be unaware of the California state policy towards medical marijuana usage? Or just, ignorant in your ways? Try keywords - medicinal, perscribed, headache, migrene, easement and try and draw a line between hemp and cocaine.
Also, it’s illegal to explicitly involve ilness(es) in minors, especially mental in nature, with substance abuse.

Drugs made by plants that are invading through extradimensional portals. They have random effects like artifacts. They could give you mutations, they could make you be able to make people instantly explode by nodding at them, or they could just make you feel really sad for about a day.
Speaking of which, cough syrup in the game should be able to cause hallucinations.
Fucking cough syrup, what was the key ingredient in that? DXM / Dextromethorphan i think. I know it’s used recreationally too, having some very silly friends who know these things firsthand.

From my personal IRL experience with acid, it should make you hallucinate but also make you very paranoid and scared. That’s how it is in real life.
I would like to see opium hit the list, too. And maybe some molly or pure X, even though it’s more a party drug, it should give you a boost akin to cocaine.
Actual psychedelic shrooms should make an appearance, too. But, again from IRL experience, you should have to eat a lot of them to hallucinate.

Well, there is poppy plants, you can make drugs out of them (painkillers and sleeping pills) shrugs

Ugh, the pink stuff has saved my stomach on more than a few occasions irl. That would be a godsend. Wouldn’t stop the stat penalties entirely IMO, but would at least keep your character from puking up their insides repeatedly and nearly starving to death as a result. I would super appreciate that.

I think.
