i forgot about my fuel running low. and got killed by stone golem and the revived zombie cows in the isolated farm i was about to make my base. I did not know zombie cows were a thing now and just passed the idea of butchering them. totally learnt my lesson.
I die due to a zombie dreadnought popping out of the water and slamming me to death. This has happened so many times due to the fact that long-neck type dinosaurs seem to be spawned more than any kind of land-based dinosaurs. In terms of the water, flying dinosaurs are everywhere and they kill all the fish and each other In seconds. I miss the old version of dino-mod where the dinos were really rare, now they outnumber everything besides the zombies.

Emerged into a research facility that had as many flaming eyes as I’ve seen in one place, they were everywhere. I had a sizable pile of loot from the lab below, but it became quite clear that it would be a miracle if I could get out with his skin intact, so I decided to abandon the loot and just try to get out. I’d been trapped there for some time and it was getting rough. I waited till night and made a desperate dash for the nearest car from the closest hole ( a hippie van). Made it to the car just as the hounds were released. One of them made it into the car through the diagonal as I was backing up and turning.
I managed to crash through the gate and died to the hound in the car just outside.
Died to compounding accidents. Was casing new city. Saw a Fungal Bloom precisely on the edge of houses, went “Nope - NOPE - N.O.P.E. Time to GTFO.” Jumped on a mojocycle, and blazed towards closest exit at max speed, weaving between zombies and vehicles. Was doing great, untill sudden Death Cam.
Apparently, there was a bank I didn’t pay attention to, and in the shadowed area behind Teller’s desks, there was a unseen Talon. It had LoS only for a single turn, since I was doing 114km/h, it must have been shooting at numerous Zs in front of the bank, but it just happened to come off CD at that precise turn, just happened to target innocent civilian driving by instead of closer horde attacking its Bank (perhaps Minneapolis cop programmed it?) and just happened to headshot me.
The lesson? No matter how great Wizard’s Hat is, and how annoying Hot Malus is, never leave your home with only flimsy piece of cloth to protect your noggin.

Haven’t actually had a character die on me yet, they’ve just been left alone when I got bored/wanted to try a new build/made a new world/updated to a new version of the game. Got really close one time though, and it was totally my fault.
Had fairly recently learned Fireball, and was wandering around town looting interesting looking places. Spotted a small group of zombies down an alleyway and decided they would make a brilliant target. Started casting, and aimed the Fireball right at the middle of them, only to have said Fireball clip the side of the building I was next to and explode - setting my head on fire.
Somehow managed to put myself out and kill the zombies, but not before I lost my miner’s helmet and ended up on one bar of head health.

My best survivor yet. Rest in peace. Killed by shocker and I had just collected a massive arsenal, some cars and done the zombie blood quest. Damn.

24 days into a wilderness start. Developed the Overly Acidic Digestion mutation, which caused her to starve to death despite consuming massive quantities of food. Her Freakishly Huge mutation didn’t help much either, as it prevented her from wearing anything other than bedding, and using any packs except rapidly decaying hide bags (which she eventually ended up eating).
Well, technically, I didn’t die because I pressed alt+f4, but my bionic soldier forgot how to throw a grenade properly and had it hit the wall right next to him instead of the wide open area I was aiming for.
I accidentaly shot myself with the grenade launcher of an APC, I was on the drivers door at the time and the mi-go scout was directly behind me by a few steps as line of sight goes… 170 points of blast damage … oops
I did not learn my lesson and almost died in the same way again.
Shrapnel, was raiding a military camp and was leading zombies through the mine field to clear it. Two birds, one stone deal. Sadly, was a bit to close and careless and some of the shrapnel killed me off.
was looting/trying to escape labs and i didnt pay close enough attention and got shot in the head by a turret, one shotting me as i had no head protection

Wrapped around a light pole.
I had cleaned out the nearby one road town long ago. I had a fully repaired, heavy V12 truck that I used as a tow vehicle, and to push over the occasional light pole to clear the path. There was one pole I’d left in place, because I could get around it pretty well, but I had to pick my way around it every time. I’d decided to push it over the next time I drove by in the big truck. I did so, but decided I’d just plow straight through it without slowing down. I figured it’d probably do a bit more damage than the usual gentle tap, but an NPC would fix it, and parts and charges were trivial. I never actually saw what killed me, but I guess that’s how it often is when you get careless.

Shot by a turret on a Military Base - wow, that thing had range!
And my next character nearly got slaughtered my dragonfly nyiads and then did get impaled by a preying mantis. I’ve been away for a while and the wildlife has definitely gotten a LOT more dangerous. Zombies are a piece of cake compare with the insects. And btw how many %$£^%%$! wasp nests are there nowadays?! I tripped over 3 within sight range of each other!
