What do the colours in radiation badges mean, mine’s green, I would also like to know if they can change overtime (when you’re close to radiation it turns red but after you go away it turns green)?
Just assume you’re gonna die throwing up in all cases.
The colours will only get more severe, they don’t revert.
Green is light doses, you might get a little sick, but with limited exposure you’re fine.
Yellow is moderate doses, get more sick, mutate a little (maybe), pop some Iodine and you should make it through.
Red is a severe dose, mutate fast, leave a horrible corpse. Don’t expect much, but Iodine tabs can’t hurt at this stage.

When you look at it in the debug menu (the non-sealed version), it says the tab is black, but I’ve never actually held on to one long enough in the game for it to turn that color.
Now I’m looking up dosimeters on wikipedia, and it seems the old film ones have largely been replaced by thermoluminescent dosimeters which glows and measures the glow when exposed to radiation.

Black is that point where you can hear the faint fizzing sound made by your own ear cells frying to death due to the radiation.
I dont think anything in game can save you at that point, you’ll die in about 12 hours.

I am 170% absolutely 300% sure that iodine tablets work by increasing the chance of your natural radiation reduction happening from infinitesimal to minimal by a few hours and that eating more than one at the same time only increases the active effect.
So you probably glitched (or weren’t using a brand new badge)
Nah wait the problem is that I play with mutations from radiation set to off, which makes radiation poisoning much more dangerous.
