This question is going to sound a little stupid, I know; how does one go about creating and adding locations to CDDA? Is there a simpler method than ASCII mapping in a text editor?

And how does loot happen? Say, for example, I made some new book items, how would I go about making those spawn in libraries and such?

To your second question: the “item\_groups.json” file and the contents of the “itemgroups” folder are what you’re after.
To the first: some digging tells me that there used to be a program made for making/modifying maps, but it no longer seems to work. So you’re stuck with text editors.
